Ischemic stroke after pellet embolization.

چکیده
A9-year-old boy was shot with a pellet shotgun and developed a visual field deficit. Head CT revealed a pellet in the left ambien cistern, in the left posterior cerebral artery on catheter angiography (figure). Chest fluoroscopy revealed multiple thoracic pellets, including a mobile cardiac pellet (video on the Neurology® Web site at Neurology.org). There was no clear cardiac injury, patent foramen ovale, or skull penetration. Arterial embolization of a pellet from the chest to the intracranial vasculature likely caused a stroke. We considered arteriotomy, endovascular retrieval, and medical therapy. The established infarct, clinical stability, and flow distal to the pellet argued for conservative treatment; the visual field deficit was unchanged at 1-month follow-up visit.

Intravenous Thrombolysis for Acute Ischemic Stroke due to Cardiac Myxoma; A Case Report
Myxoma may cause systemic embolization and frequently presents as ischemic stroke. There has been debates whether it is safe to use recombinant tissue plasminogen activator (rt-PA) in patients with cardiac myxoma who are presented with ischemic stroke at emergency department. we describe a young case of atrial myxoma with initial presentation of acute cerebral infarction symptoms who was treate...
متن کاملDelayed Ischemic Stroke after Flow Diversion of Large Posterior Communicating Artery Aneurysm
For securing large, giant, and wide-neck aneurysms, conventional coil embolization has substantial limitations, such as incomplete occlusion, recanalization, and a high recurrence rate. To overcome these limitations, a novel paradigm was suggested and, as a result, flow-diverting device was developed. The flow-diverting device is an innovative and effective technique to allow securing of large,...
متن کاملOnset Time of Ischemic Events and Antiplatelet Therapy after Intracranial Stent-assisted Coil Embolization.
BACKGROUND Stent-assisted coil embolization is effective for intracranial aneurysms, especially wide-necked aneurysms; however, the optimal antiplatelet regimens for ischemic events that develop after coil embolization have not yet been established. We aimed to determine the onset time of such postoperative ischemic events and the relationship between these events and antiplatelet therapy. ME...
متن کاملImpact of arterial reocclusion and distal fragmentation during thrombolysis among patients with acute ischemic stroke.
BACKGROUND AND PURPOSE Arterial reocclusion and distal embolization are known complications of ischemic stroke intervention, impacting treatment strategies and device design. We sought to determine their rates of occurrence and effects on long-term outcomes during endovascular treatment of patients with acute ischemic stroke. MATERIALS AND METHODS Retrospective analysis of data from 4 prospec...
متن کاملShotgun pellet embolization to the posterior cerebral circulation.
The embolization of missiles to the intracranial circulation is a rare event. We describe here a case of a man shot by a shotgun, presenting with a posterior fossa stroke and occlusion of the superior cerebellar artery by a metallic pellet. To our knowledge, this is the third case of posterior fossa circulation pellet emboli.
